MINISTRY and ACTIVITIES EVENT REQUEST FORM

The Ministry and Activities Event Request Form is used for requesting to promote or use the church for a ministry and or activity. This is for all departments within the church and for church attenders to request onsite and offsite activities to be placed on the master calendar. Please fill out the following form in order to request building space, personnel, a/v equipment, etc. One of our staff members will be in touch with you regarding your event and whether or not we can fulfill your request. Please fill out every field and give us as much information as possible.
